Basic Disease Management Programs
Scott & White Health Plan members are eligible for VitalCare basic disease management programs if they meet the criteria for each specific disease management program. (VitalCare is part of the member's contract.) Eligible members automatically receive program information in the mail. If a member needs additional information or would like to "opt out" of any program, they may call Scott & White Health Plan, Quality Improvement Division toll free at 800-321-7947 ext. 3097 or local Temple phone number 254-298-3097.
Scott & White Health Plan has the following basic disease management programs available for Health Plan members at no cost.

VitalCare Lifestyle Management Programs
A variety of lifestyle management programs are available by going to the Basic Wellness and Disease Management page. Disease Management Program participants are invited to take a Health Risk Appraisal (VitalCare Succeed®) which results in tailored and targeted information and tools regarding lifestyle management.
The VitalCare CareTM for Your Health module is tailored to help manage long-term health issues including:
- How to manage common symptoms
- Establishing a routine for better medication management
- Sticking with your treatment plan
- Coping with daily challenges and disruptions
- Strategies for helping you deal with stress and worry
- Practical tips for getting a good night's sleep
- Getting the support you need from family and friends
- Learning how to talk with your doctor to get the answers you need
- Understanding how your pharmacist can become a key part of your treatment team

Asthma
The objectives of the Asthma Program are to improve coordination of care for asthma patients of all ages and to enhance quality of life by encouraging and empowering members with asthma to participate in managing their own health. Members are provided pertinent information, tools, training and care management support.

Secondary Preventive Coronary Artery Disease
This program focuses on lipid management, blood pressure control, nutrition, smoking cessation, beta blocker and aspirin therapy.
- Members are provided an opportunity to request educational information.
- Members receive a reminder letter in the first [1-2] months after an event, suggesting they make an appointment with their Primary Care Practitioner (PCP) to have a lipid measurement done, if not already done in the [365] days prior to event.
- Members receive a quarterly Coronary Artery Disease Bulletin with patient education and life-style management information.

Diabetes
Members age 18 to 75 are eligible for the Diabetes Program. Objectives are that members will obtain important exams including blood pressure, foot exam, eye exam with ophthalmologist or optometrist and obtain follow-up care by primary care physician, including monitoring of medication and laboratory tests results (hemoglobin A1c, lipid profile and micro albumin). Other health conditions are considered such as hypertension and kidney complications.
- Members are provided an opportunity to request educational information.
- Members receive reminders regarding recommended exams and tests.
- For condition monitoring, members receive a tool to document and track blood pressure readings for easy reporting to their primary care physician.
- For treatment plan adherence, members' primary care physicians receive alerts when tests results are out of control or tests have not been completed in the last 12 months.

Congestive Heart Failure
- Members age 18 to 75 are eligible for the Congestive Heart Failure (CHF) Program.
- Members receive educational information regarding CHF.
- Members are provided an opportunity to request additional educational materials related to heart disease.
- Members receive a tool to document and track blood pressure readings for easy reporting to their primary care physician.
