Scott & White Health Plan

Eligible participants must:

• Live in the Scott & White Health Plan's SeniorCare (Cost) HMO Plan service area  
• Is entitled to benefits under Medicare Part A and enrolled in Medicare Part B
• Is enrolled in Medicare Part B only. If you do not have Medicare Part A, you can purchase it from Social Security or Scott and White Health Plan. 
• Not have End Stage Renal Disease**

Eligible prescription participants must:

• Live in the Scott & White Health Plan's SeniorCare Rx (Cost) HMO Plan service area
• Be enrolled in Senior Select, Senior Preferred, Senior VIP, Senior MedOption, or Senior Premium 

If you do not have Medicare Part A, you can purchase it from Social Security or Scott & White Health Plan.

You can enroll in medical coverage: 

• When you are entitled to benefits under Medicare Part A and are enrolled in Medicare Part B
• During open enrollment. Open enrollment is held annually during October 15th through December 7th of each year.
• If you qualify for a special enrollment period.

You can enroll in prescription coverage:

• When you first become eligible to enroll for Medicare
• During open enrollment. Open enrollment is held annually during October 15th through December 7th of each year.
• If you qualify for a special enrollment period
• The SeniorCare (Cost) HMO plan prescription plan is optional and only available to SeniorCare (Cost) HMO plan members.
